DAPA CHECKLIST
Evidence that you are the parent of a United States citizen or Legal Permanent Resident since 11/20/2014:
- Birth certificate
- Naturalization certificate
- Permanent legal resident card
Evidence of continuous physical residence, from 01/01/2010 to the PRESENT, and evidence that you have been present in the United States when the announcement was made known on 11/20/2014 (Must contain your name and the date of issuance of the evidence)
- Record doctors, hospitals and/or clinics, dentist,
- University record, school record
- First communion and/or baptism documents in the United States
- Utility and public service receipts, telephone bills, cell phone bills, gas, etc.
- Bank account statements, credit card receipts, remittance receipts
- Records of the gym, library, or other businesses where you were a customer (costco, laundry...)
- Warehouse collection receipts
- Paychecks
- Copies of envelopes addressed to you with the postage stamp date
- Records of unions, associations or group memberships with dates
- Work certificates showing presence at work
- Travel records
- Hotel payments
- Church records confirming membership and attendance
- Rent payment receipts/rental contract
- Car registration/insurance/car insurance
- Medical insurance
- Loans or mortgages
- Letters from the school certifying attendance at events, from teachers, coaches
- sports
- Taxes
Record of previous contact with the police (arrest, ticket, conviction): IMP CONSULT WITH A LAWYER!
- Court records (final court disposition)
- Police reports
- Results of your criminal check (FBI)
Record of previous cases with immigration. IMP CONSULT WITH A LAWYER!
- Immigration court record (Notice to appear, deportation order, court summons)
- Copies of immigration applications that someone has made for you
- Copy of visa/I-94 if I enter with a visa
Letters of good moral character/good reputation (Not necessary but preferable)
- From the parish priest/pastor of the church,
- of boss
- Teachers or school principal, community organizations
- Copies of awards, distinctions, community, sporting, cultural or academic honors

- Not be a priority for immigration, for example: gang members, or people who have committed visa fraud, and who have committed felonies, or a significant misdemeanor or multiple misdemeanors and not be a threat to the national security or public safety of the States Joined
